The decision tree should separate five distinct pathways:

1. Immediate protection : control the risk where safety, water ingress or asset exposure requires urgent action.
2. Investigation : establish the condition, cause, extent and contractual or regulatory consequence before selecting a remedy.
3. Rectification : implement an approved corrective action supported by verified evidence and defined acceptance criteria.
4. Acceptance by concession : only where the deviation is understood, authorised by the appropriate decision-maker and supported by documented risk evaluation.
5. Redesign : where the original solution is no longer viable, compliant or fit for purpose.

Calling an inspection “independent” does not make it independent.

A meaningful verification process considers:

• Who appointed the reviewer
• Who pays for the review
• What information is available
• Whether the reviewer can access concealed work

The hidden risk is not simply a missed booking. It is a liability vacuum created when a test is scheduled after the evidence window has closed, or when its outcome arrives too late to influence the work.

This is not about delaying payment. It is about ensuring payment reflects an understood and documented position.

A test result is only as defensible as the custody record connecting it to the installed work.

This is not simply material traceability. It is evidentiary continuity. Without a reliable custody record, a laboratory result can become detached from the wall, slab, membrane or other construction element it is intended to represent; the result may remain technically valid, yet its project relevance becomes vulnerable to challenge.

A defensible process should establish:

1. Who selected the sample, and under what instruction.
2. How the sample was uniquely identified and linked to a precise location.
3. Who maintained control during storage and transport.
4. When the sample was received, tested and reported.
5. Whether any transfer, delay, damage or discrepancy was recorded.

Human oversight remains essential. Digital records support the process; they do not replace competent judgement, observed conditions or accountable sign-off. The objective is a Safe Space for decision-making, where forensic evidence can withstand technical, contractual and legal scrutiny.

Concealment is not a routine construction sequence; it is a controlled release of evidence.

Before work is covered, the project team should confirm five matters:

1. Inspection status: has the required inspection occurred, and has its outcome been recorded?
2. Outstanding evidence: are certificates, test results, approvals or clarifications still pending?
3. Photographic requirements: has the concealed work been documented at the required level of detail?
4. Access implications: will future verification, maintenance or dispute assessment be compromised once the work is enclosed?
5. Authorised release: has a suitably responsible person formally permitted concealment?

The critical safeguard is human oversight. A checklist cannot determine whether the evidence is sufficient, whether an unresolved issue carries material risk, or whether release would create a liability vacuum.

A disciplined Safe Space allows concerns to be raised before visibility is lost: not after the wall, ceiling or lining has converted a reviewable condition into a forensic uncertainty.
