AFFIDAVIT OF LOSS

I, Diosdado C. Perater, of legal age, Filipino, and a resident of Lot 9 Block 12 Northwinds Phase 8 Subd, Sto. Cristo, San Jose del Monte City, Bulacan, after having been duly sworn under oath, hereby depose and say:

1. That I am a subscriber of a postpaid sim card no. 09321105439 (Sun);

2. That I lost my cellular phone when I was about to aboard an E-jeep in Palmera Homes, Quirino Highway, Santo Cristo, San Jose del Monte City, Bulacan, last October 10, 2022;

3. That I notice the lost when I arrived at Novaliches Market, Quezon City on the same day;

4. That despite diligent efforts exerted to locate the same, I could no longer find, hence considered permanently lost;

5. That I am executing this affidavit to attest to the truth of the foregoing allegations for securing new sim card with the same number in lieu of the lost one.

4. To transact with the City Assessor’s Office, Treasurer’s Office, Bureau of Internal Revenue District Office and Registry of Deeds for the payment of Taxes, fees and other charges pertinent to my purchase of the aforementioned property covered; and;

5. To do such any other act that may be required necessary or incidental to carry out effectively any and all of the purposes for which this authority is hereby given.

HEREBY GIVING AND GRANTING unto my attorney full power and authority to do and perform all and every act and thing whatsoever requisite and necessary to be done in and about the premises, as fully, to all intents and purposes, as I might or could do if personally present with all power of substitution and revocation, and hereby ratifying and confirming all that my said attorney shall lawfully do or cause to be done by virtue thereof.
