[for all seafarers regardless of origin]

Where: Pasig Sports Complex
Date: 28 August 2021
Time: 8:00 am to 5:00 pm
Vaccine Brand: Moderna

There are 700 doses available for 28 August 2021. Registration is on a first come, first served basis.

On their appointment date, seafarers have to bring the following:
1. Seaman’s book or SID;
2. Ticket/Entry pass with QR code;
3. Ballpen; and
4. Alcohol.

IMPORTANT REMINDERS:
1. Register for Pasig QR Code online as Visiting Person:
https://pasigpass.pasigcity.gov.ph/
2. Check A1 Group (OFWs) or A4 category
3. Get a Waiver Form
