ODSP APPLICATION FOR SY 2022-2023 IS NOW OPEN!

HOW TO APPLY FOR ODSP SY 2022-2023

1. Go to tinyurl.com/24voduk4 for the “ODSP SY 2022-2023 ELIGIBILITY CHECKER” and answer the questions correctly and truthfully.

2. If you are ELIGIBLE TO APPLY for ODSP, a link to the ODSP application form with a LIST OF REQUIREMENTS will be given. Download, print(legal size, 8.5x13), and fill out the application form.

3. Eligible applicants for REGION 3 must submit their applications with complete documentary requirements at the OWWA Regional Welfare Office 3 at Matulungin St., cor Mahusay St., Diosdado Macapagal Government Center, Brgy Maimpis, City of San Fernando, Pampanga.

Note: Applications with INCOMPLETE/INCORRECT documentary requirements will not be accepted.

Filing of application for OWWA Regional Welfare Office 3 (REGION 3) is from June 27, 2022, to September 9, 2022.

Office hours are from 8am-5pm, Monday to Friday except for holidays.

1. Magpunta sa tinyurl.com/24voduk4 para sa “ODSP SY 2022-2023 ELIGIBILITY CHECKER” at sagutin ng tama at totoo lamang ang mga katanungan.

2. Kung ikaw ay KWALIPIKADO NA MAG-APPLY SA ODSP, bibigyan ka ng link sa ODSP application form na mayroong listahan ng lahat ng requirements. Idownload, print(legal size, 8.5x13), at sagutan ang application form.

3. Ang mga kwalipikadong aplikante sa REGION 3 at kailangan magsubmit ng kanilang application form na may kumpletong dokumento sa OWWA Regional Welfare Office 3 at Matulungin St., cor Mahusay St., Diosdado Macapagal Government Center, Brgy Maimpis, City of San Fernando, Pampanga.

Paalala: Ang mga aplikasyon na may kulang o maling dokumento ay hindi tatanggapin.

Bukas ang aplikasyon sa OWWA Regional Welfare Office 3 (REGION 3) simula June 27, 2022, hanggang September 9, 2022.

Sa mga iba pang katanungan, maaari tumawag sa 0956 692 4338.

Ang opisina ng OWWA RWO3 ay bukas mula 8am hanggang 5pm, Lunes hanggang Biyernes except sa mga holidays.
