Nanay, tatay, at guardians, bigyan ng dagdag proteksyon ang inyong mga mahal na anak na nasa 5 - 11 taong gulang, ito na ang tamang panahon para pabakunahan sila kontra COVID-19.
Ang bakuna kontra COVID-19 para sa mga batang edad 5 - 11 ay APRUBADO, LIGTAS, EPEKTIBO, at LIBRE.
Ito ay magbibigay proteksyon upang makaiwas sa malalang sintomas na maaaring maging sanhi ng pagka-ospital o kamatayan dahil sa COVID-19.
MAGPALISTA NA SA INYONG MGA BARANGAY HEALTH STATIONS.
Dahil limitado pa ang ating suplay ng bakuna para sa ead 5 – 11, HINDI MUNA PINAHIHINTULUTAN ANG WALK-INS sa ating mga vaccination site.

𝗗𝗔𝗣𝗔𝗧 𝗔𝗬 𝗞𝗨𝗠𝗣𝗟𝗘𝗧𝗢 𝗔𝗡𝗚 𝗥𝗘𝗤𝗨𝗜𝗥𝗘𝗠𝗘𝗡𝗧𝗦.
Dapat rin ay kasama ng vaccinee ang kaniyang MAGULANG o GUARDIAN.
𝗔𝗡𝗢 𝗔𝗡𝗚 𝗠𝗚𝗔 𝗞𝗔𝗜𝗟𝗔𝗡𝗚𝗔𝗡𝗚 𝗗𝗔𝗛𝗜𝗟 𝗦𝗔 𝗔𝗥𝗔𝗪 𝗡𝗚 𝗕𝗔𝗞𝗨𝗡𝗔?
• Dalhin ang mga sumusunod:
- Dokumentong magpapatunay ng relasyon ng anak sa magulang.
Halimbawa: Birth Certificate, Baptismal Certificate
- Valid ID ng batang babakunahan (may litrato)
- Valid ID ng magulang/guardian (may litrato)
• Medical Certificate para sa mga batang may sakit
• Magsuot ng face mask

𝗠𝗮𝗴𝗱𝗮𝗹𝗮 𝗹𝗮𝗺𝗮𝗻𝗴 𝗻𝗴 𝗸𝗮𝗵𝗶𝘁 𝗜𝗦𝗔 𝘀𝗮 𝗺𝗴𝗮 𝘀𝘂𝗺𝘂𝘀𝘂𝗻𝗼𝗱:
• Original copy or CTC of birth certificate
• Secondary IDs (with parent + child names):
- Authenticated medical certificate of the child bearing name of the parent, issued by hospital or DOH;
- Baptismal Certificate of the child with the name of the parent/s;
- School ID or records of the child (transcript, Form 137, etc.) bearing the name of the parent;
- PhilHealth, Social Security System (SSS), Government Service Insurance System (GSIS) forms, insurance policies, health card membership, life or memorial plans, etc. (indicating that the vaccine recipient is a beneficiary and a child of the parent)
- Barangay Certification issued by the Barangay Captain indicating that the parent/s and the child is personally known + attested by one (1) other witness who personally knows the child and the parent;
- Solo Parent Identification card from the City or Municipal Social Welfare and Development Office;
- Court Decree of Adoption, in case the child is adopted;
- PWD ID of the child
- Other public documents issued by the Office of the Civil Registrar General, as applicable.

1. May medical complexity: nakasalalay sa makinarya tulad ng tracheostomy; may developmental delay o genetic anomaly
2. May genetic conditions: Down Syndrome, G6PD, primary immunodeficiency disorders, thalassemia at iba pang chromosomal abnormality
3. May neurologic conditions: seizure disorder, autism spectrum disorder, cerebral palsy, stroke in the young, chronic meningitis tulad ng tuberculosis, chronic neuromuscular disease, chronic demyelinating disease
4. May metabolic/endocrine disease: diabetes mellitus, diabetes insipidus, hypothyroidism, adrenal insufficiency, hypopituitarism, at iba pang namamanang metabolic disease
5. May cardiovascular diseases (sakit sa puso): hypertension, congenital heart disease, cardiomyopathy, rheumatic heart disease, mitral valve disease, pulmonary hypertension with right heart failure
6. Obese: BMI > 95th percentile for age and height
7. Na-diagnose ng HIV
8. May tuberculosis: pulmonary, extrapulmonary, and disseminated (involvement of 2 or more organs)
