A double tap occurs when two wires are connected to a single circuit breaker that is only engineered to accept one wire. This is a defect because two wires in a single breaker may not be properly tightened to the breaker and loose wires have the potential to cause overheating and arcing. Also, too much electricity could be drawn through the breaker and potentially cause it to trip.

When inspecting an attic you never want to see holes cut into the wood. Someone was either cutting out wood rot, or maybe there was a vent pipe that wasn't replaced. Either way you can see there is water intrusion and this needs to be properly repaired.
