This document was hard to find because you'll note it has the wrong name (Hugh McWhorter) at the top. However, it is signed by my Great Grandfather, Henry Ni**od McWhorter, at the bottom. While serving in the Confederate Army, was captured by Union forces near Decatur, AL. He was imprisoned at Camp Chase, Ohio. I know this is Henry's signature because it was combined with his other release papers.

By virtue of an Order from The President dated at Washington City D.C. December 1st 1865,
Hugh McWhorter Pri Employed in Nitre Works resident of Lawrence County,
and State of Ala, Prisoner at this Post; after
having complied with the requirements of, and subscribed the following Oath
and Declaration is hereby released from confinement:

UNITED STATES OF AMERICA.
I, Hugh McWhorter Resident of the County
of Lawrence State of Alabama do solemnly swear that
I will support, protect and defend the Constitution and Government of the United States
against all enemies, whether domestic or foreign; that I will bear true faith, allegiance
and loyalty to the same, any ordinance, resolution, or laws of any State, Convention, or
Legislature, to the contrary, notwithstanding; and, further, that I will faithfully
perform all the duties which may be required of me by the laws of the United States;
and I take this oath freely and voluntarily, without any mental reservation or evasion
whatever.
[Now handwritten I believe it says] (?) I furthermore Pledge My Oath of Honor to remain North of the Ohio River during the existence of the present Rebellion.
