I just completed a course called Causes of Foundation and Structural failure. Its main focus was to educate about how to identify failures and determine the causes of failures. The majority of failures occur from water or hydrostatic pressure or poorly constructed foundations and materials.
Pictured here is a 100 year old Terra cotta basement wall which is very porous.

This foundation crack started on the exterior of the home. The crack went across the top of the foundation and traveled down the interior basement wall and ended at the floor
