LifeStream is proud and grateful for the well-established Community Partnerships we have with many of the Law Enforcement Agencies throughout Citrus, Hernando, Hillsborough, Lake, Marion, Orange and Sumter counties.

UPDATED NOTICE AS OF 09/02/19 @ 1100 HRS: Due to the most current information received from the National Weather Service, LifeStream will resume normal operations at our Citrus and Sumter County facilities on Wednesday morning. Another update is scheduled for 09/03/19 @ 1100 hrs and a determination will be made regarding operations at our Lake and Orange County facilities.
CONSUMER NOTICE: Due to the approaching hurricane, LifeStream’s Administrative and Outpatient offices center-wide will be closed on Monday and Tuesday, September 2nd and 3rd. In case of an emergency, please call our toll free 24hr crisis hotline at 1-866-355-9394.
EMPLOYEE NOTICE: In anticipation of Hurricane Dorian’s expected impact on Central FL, LifeStream will close all Outpatient and Administrative offices Tuesday, Sept. 3. Please contact our Human Resources line at 352-315-7515 for more information or your Direct Program Supervisor.
