My name is Kim Conner, and I am the founder of Hammock Coast Senior Transitions. I help seniors and their families navigate one of life’s most meaningful — and often overwhelming — transitions: recognizing when additional support is needed and understanding the steps involved in moving into senior living.
My work is rooted in both professional experience and personal understanding.
I spent more than 21 years in healthcare, including 14 years working closely with seniors in a hospital setting. During that time, I gained a deep understanding of the physical, emotional, and logistical challenges that often arise as we age — not only for seniors themselves, but for the families supporting them.
On a personal level, I have been a caregiver to three parents through end of life. Those experiences shaped my compassion, patience, and ability to truly listen. I understand how emotional these decisions can be and how important it is to feel supported, informed, and respected every step of the way.
My education includes studies in Ethics in an Aging Society at Virginia Tech, reinforcing my commitment to dignity, advocacy, and thoughtful decision-making for older adults.
In addition, I have spent the past 10 years as a Realtor, working with individuals and families at every stage of life — from first-time homebuyers to retirees downsizing or relocating. I understand how housing decisions intersect with health, finances, and family dynamics, especially during times of transition.
My strength lies in combining healthcare experience, life experience, and real estate expertise to serve as a trusted resource for seniors and their loved ones. Whether you are just beginning to explore options or facing an immediate need, my role is to provide clarity, guidance, and steady support — without pressure.
At Hammock Coast Senior Transitions, my goal is simple:
to help you navigate this chapter with confidence, compassion, and peace of mind.
