If you have questions about how to participate, please contact the office and we will be glad to help!

Please note that your participation in completing the My Life Check® assessment is entirely optional. You may choose not to participate in completing this assessment, and you will not be penalized for choosing not to participate. Your registration email will be used solely to confirm eligibility to participate in the raffle drawing and will not be used or retained for any other purpose.
