Nobody gets a violation because they got unlucky. They get it because of a decision that felt small the day it was made.
Most of the time it starts with a permit that was never pulled. The wiring might have been done correctly. The city does not care. It does not inspect your walls by feel. It checks whether a filing exists. No filing means no proof the work ever happened the right way, and no proof means a violation, whether the outlet works perfectly or not.
The same is true of who did the work. A skilled friend, a cheap contractor, a guy who does good work off the books. None of that matters to the Department of Buildings. Only the license attached to the filing exists in their system. Good work by the wrong person is still illegal work.

They just require you to still be there when the system runs its course.
This is why it always seems to surface at the worst possible time. A buyer's title search. A new contractor pulling a permit and finding the old one was never closed. A refinance that stalls because of paperwork from years ago that nobody dealt with. The problem was never gone. It was only quiet.
Avoiding a problem does not make it disappear. It just waits for the moment you can least afford it.
