Something shows up on the inspection report that the seller could reasonably cover, and the buyer decides not to bring it up because the deal is moving and they don't want to be the reason it stops moving. This comes up constantly with veteran buyers, and it usually costs them money they were never getting back.

What gets missed is that the seller was already expecting a counter. Their agent priced the house assuming somebody would ask about repairs or help with closing costs, so when nobody asks, the seller just keeps that margin and nobody sends a thank you note for it.
